1. Use the Adobe Acrobat Reader DC link below to ensure you have the latest version.

2. Click on the print form and download to your computer.

3. After filling the form in, 'Save' or 'File' and 'Save As.'

4. Finally, click the envelope on the toolbar to email the form.
